COMPLAINTS + GRIEVANCES

The NICA Accreditation Program will address all complaints and grievances received against an organization currently accredited by NICA or against the NICA Accreditation Program or personnel. Complaints and Grievances may be submitted via mail, email, fax, phone, or online. Online submissions must be submitted by utilizing this Complaint Submission Form. NICA takes all complaints and grievances seriously, and it is the NICA Accreditation Program policy to ensure all complaints and grievances are handled confidentially and securely. Please do not submit any personally protected health information (PHI), such as social security number or date of birth. The submission of PHI is strictly prohibited.
